Dates, times, and descriptions help you show a pattern of behavior to a court, advocate, or counselor. Notes about witnesses or reports can also support safety planning and documentation.
What to record
- When it happened: the date, approximate time, and location.
- What happened: threats, physical contact, property damage, or coercive behavior.
- Injuries or damage: bruises, medical care, broken items, or photos you collected.
- Who saw or heard it: names of witnesses, neighbors, or children present.
- Reports you made: police report numbers, CPS notices, shelter intakes, or school contacts.
